Majority of the patients with cancer receive external beam radiation therapy at some stage of their illness. Radiotherapy can be employed as a curative treatment, as a method of tumour control or for management of cancer related symptoms. It may be given alone or in combination with another treatment such as chemotherapy or surgery. Skin is vulnerable to the effects of radiation due to its continuous state of cellular renewal. Patients receiving radiotherapy are at risk of experiencing skin reactions because the radiation beams must pass through the skin surface. Many patients suffer from acute radiation induced skin reactions which develop around 2- 3 weeks of radiotherapy and persist for upto 4 weeks after the treatment has finished. Erythema, dry desquamation and moist desquamation are the most commonly seen reactions.1

            Radiation induced skin reaction is a common side effect of radiotherapy, despite the introduction of sophisticated skin sparing techniques.   Radiation induced skin toxicity is a prominent clinical problem that affects majority of the patients receiving radiation therapy. Concomitant chemotherapy and radiotherapy can further increase the risk of an adverse reaction. 9Reactions vary from mild erythema to moist desquamation. Severity of reactions depends on both intrinsic and extrinsic factors. Intrinsic factors include demographic or disease related characteristics such as age, infection, hormonal status and coexisting disease. Extrinsic factors are treatment related factors such as   radiation fractionation, total dose, anatomic treatment area, radiation energy, and adjuvant chemotherapy.1 severe skin reactions are painful, may lead to localized and occasionally systemic infection and may cause permanent scarring. At times, radiation induced skin toxicity may require temporary or permanent treatment cessation. 4

          The Aloe Vera plant is a member of lily plant which is full of juice and similar to a cactus of which there are 360 species. The most beneficial species of aloe vera plant is Aloe Vera Barbadensis. The leaves of Aloe Vera plant contain gel, 96 percent of water and about 4 percent of other substances. Aloe vera contains 75 potentially active constituents such as minerals, sugars, lignin, saponin, salicylic acids and aminoacids.2

          Aloe Vera provides number of benefits to the health of the person .The different uses of aloe vera is:-

Aloe Vera contains various essential nutrients like minerals, vitamins and enzymes which help in strengthening the immune system. Another health benefit of aloe vera is that aloe vera juice is used in relieving digestive problems, heartburn, irritable bowel syndrome and ulcer. Aloe vera gel is helpful in case of minor cuts, scrapes, bruises, burns and Psoriasis by reducing the itching, swelling  and pain. The extracts of aloe vera are used in lowering blood sugar levels and aloe vera gel provides beneficial effect in reducing cholesterol and blood fat lipid levels. Also effective in asthma, chronic fatigue syndrome, crohn’s disease and ulcerative colitis.

Aloe Vera and cancer

Aloe Vera has been demonstrated to enhance the immune system's response to cancer, promote the growth of new and healthy cells, and reduce the overall viral load within the body thereby revitalizing the body in its fight against the cancer. When radiation and chemotherapy are necessary, Aloe vera helps to minimize the damage done to the body by these treatments, which destroy healthy cells - particularly immune system cells - crucial to the body's recovery. The use of Aloe Vera enables the body to heal itself from cancer and the damage done to it by conventional treatment: the immune system is boosted, tumors shrink, metastases are reduced so the cancer does not spread, and new healthy cells begin to grow. There is clear scientific evidence that, in vitro, Aloe Vera will suppress cancer tumor growth, stimulate immune system response, raise tumor necrosis levels, and promote healthy tissue growth . 3  

Patients should apply Aloe Vera to the site of head and neck radiation therapy at least twice a day starting from the first day of radiotherapy throughout the treatment period and 2 weeks after radiation therapy. Aloe Vera application should be preserved for at least 15- 30 minutes without any covering. Aloe Vera should be applied to the site of radiotherapy twice a day (in the morning, in the night). Patients should apply Aloe Vera 1 - 2 hr before radiotherapy & another application in the night. Total application should be of minimum 12 hr (contact period).  One diary should be given to the patients to mention about time of application, at what time they washed & record of any side effects (allergic reaction).

     Aloe Vera gel is available in the market with the following indications on the label anti inflammatory  ,easily penetrates into skin, gives soothing & healing effect from UV burns & chemical burns. It is multipurpose good for eczema, psoriasis, cuts, burns, scraps, sun allergy & other major irritations. (Dr. K.K.’s Be Sure Aloe Vera skin gel, cost: for 100 mg 70 Rs.) . For this study Aloe Vera gel will be procured from Dr. K.K.’s Be Sure Aloe Vera Company and provided to them.
